Section One
Your Starting Point
Your answers here decide which sections you see below.
Sensitive Information Needed
Because TIH is filing your business registration or your EIN, the IRS and your state require the business owner's Social Security Number. We do not collect it through this form. It goes into the private, shared Google Drive folder we create for you, the same secure folder where your logins are kept. We will walk you through that at Kickoff Part 1.
Your Engagement Scope
Answer the questions above and your scope appears here.
Section Two

Section Three
Business Email, Domain, Address, and Phone
All four are required for every build. For each one, tell us where it stands, then give us the details that follow. We need the actual names and numbers whether TIH sets it up or you do.
Why all four: A business email on your own domain, a business address, and a business phone number keep your business legally separate from your personal life. That separation is what protects you. Our CRM also can't send campaigns, newsletters, or automations from a personal Gmail, Yahoo, Hotmail, or Outlook.com address. Without a professional email and domain, the build cannot start. Any service that isn't in your signed proposal is quoted separately, and TIH invoices you before that work begins.
Your Domain
Your domain is your business's web address, like yourbusiness.com. Your website and your business email both live on it.
Where does your domain stand?
Select Included in my proposal Requesting as an added service I already have it Handling it myself before kickoff
Access we'll need: TIH manages the domain settings (called DNS) for the length of your build so your website and email point to the right places. Access to your domain account is handed off at your Setup and Workspace kickoff.
Your Professional Business Email
This is an email address on your own domain, like you@yourbusiness.com. Personal addresses will not work in your build.
Where does your business email stand?
Select Included in my proposal Requesting as an added service I already have it Handling it myself before kickoff
Your Business Address
The address your business uses on filings, your website, banking, and mail. A virtual address is a real street address a service runs for you so your home address stays private.

Your Business Phone
A number that belongs to the business, separate from your personal cell. It goes on your website, listings, and forms.

Section Four
Your Existing Business
What's already filed. We match everything we build to it.
Business Type How your business is set up with the state. Check your filing paperwork if you're not sure.
Select Limited Liability Company (LLC) S Corporation C Corporation Sole Proprietor (no company filed, just me) Partnership Nonprofit Not sure
Section Five RUNS FIRST
Business Registration
Registration runs before anything gets branded. We can't build around a name that hasn't been legally secured. Filing begins within 3 business days of intake receipt, or once your add-on quote is accepted and paid.
About your business name: Give us three choices in order. The state rejects a name if another business already has it, so backups keep your filing moving. We run availability and trademark checks before we file.
Business Name: First Choice
What Your Business Does One or two plain sentences. This goes on your public state filing. Example: "Life insurance and retirement planning services for families."
Business Tax Year Most small businesses use the calendar year. Choose that unless your accountant told you otherwise.
Calendar Year (Jan-Dec) Other (specify in notes)
Section Six
Your Business Name (Doing Business As)
You're launching without filing a company right now. Here's how we set you up to operate anyway.
What this means: Until you file a company, you're a sole proprietor: the business is you, under your own name. To operate under a business name (on your website, your email, your bank account) you need a "doing business as" name, called a DBA, filed with your state or county. TIH sets you up with a DBA, so give us the name you want. We check availability before we brand anything. Nothing else gets filed on this path, and the decision to file a company later stays with you.
Business Name (DBA): First Choice
The Service You're Launching One or two sentences. What you'll sell and to whom.
When do you plan to file a company?
Select Within 90 days Within 6 months Within 12 months Undecided
Section Seven
Your Industry and Any Licenses
This tells us what rules your build has to respect. Some industries limit what you can say in marketing. Everything we build stays inside those lines.
Professional Licenses You Hold Each license, the state, and the number. Example: Texas Life License #12345. Enter "None" if you don't hold any.
Company You Work Under or Are Appointed With (if any) Example: your brokerage, your insurance carrier or marketing organization, your franchise. Enter "None" if you're fully independent.
Words or Claims You're Not Allowed to Use Anything your license or company restricts in advertising. Example: can't say "investment advice," can't promise guaranteed returns. Enter "None" if nothing applies.
Section Eight
Brand Direction
Your logo, colors, and voice all come from your answers here. Be specific. There are no wrong answers, but vague answers slow the build.
In two to four sentences, describe what your business does and who it serves.
Describe your ideal client in one paragraph. Age range, life stage, income, values, what they're trying to solve.
What makes you different from others in your industry?
Color Preferences Colors you love, colors to avoid, any colors you already use.
Brands or Professionals You Admire Two or three whose look and feel you'd want yours to sit alongside. Links welcome.
Section Nine
Website
What your site needs to do and what already exists.
What the Site Should Say and Do Brief notes. Example: a headline, an about section, your services, a contact form, a booking link.
Section Ten
Your Social Media and Business Listings
Tell us where you stand on each platform so we know what to build and what to fix. Your build syncs up to three. Mark anything you don't have and don't want as not applicable.
What we need from you: For each platform you already have, give us the profile link and the email address the account is registered under. Access to the accounts is handed off at your Setup and Workspace kickoff, not through this form.
Any Other Platforms Name, link, and account email for each. Example: Threads, Pinterest, Nextdoor, Alignable.
Section Eleven
Services and Offers
This shapes what your lead page, booking form, and follow-up emails actually say.
Top Three Services You Offer or Are Building Name each and add one sentence.
The One Thing You Want a Visitor to Do Example: book a free consultation, buy a discovery session, sign up for a workshop, request a quote.
Days and Times You Take Client Appointments Include your time zone. This gets built into your booking page.
Section Twelve
Your Contacts and Backend Systems
What you run on today, what moves over, and what starts fresh.
What a CRM is: A customer relationship management system (CRM) is one place that holds every contact and lead, tracks where each one is in your process, and sends follow-up automatically. Right now yours might be a spreadsheet, your phone contacts, or nothing at all. That's fine. Tell us what's true.
How a New Lead Moves Through Your Business Today In your own words, step by step. Example: they reach out, we talk, I send a proposal, they pay, I onboard them.
What You Want to Happen Automatically Example: a new lead gets a text and an email within five minutes, an invoice sends after booking, a reminder goes out 24 hours before appointments.
Section Thirteen
Your AI Workflow
Your Foundation Build includes one AI workflow: a tool set up to handle one repeat task for you, built to stay inside your industry's rules. Pick the task that saves you the most time.
What It Must Never Say or Do Example: never promise a rate of return, never quote a price. Enter "None" if nothing applies.
Section Fourteen
Deadlines and Priorities
Any Hard Deadlines During Your Build? Example: a speaking engagement on October 20. Give the date and the event. Enter "None" if nothing is fixed.
The One Deliverable You Want Live First
Anything Else You Need That We Haven't Asked About If it's not in your signed proposal, we quote it separately. Better to surface it now than mid-build.
Section Sixteen
